If you are interested in joining the Toledo Craftsman's Guild we will need your,

First & Last Name, Address, City, State, Zip Code, Phone Number and Email address.

We also will need three pictures of your craft and one picture of your display.

Pictures must be 4in x 6in in size clear and in color. (INCLUDE THESE WITH YOUR EMAIL INFORMATION)
